New Patient Information

First Visit Information

During your first visit with us, we will go through your entire health history. We kindly ask that you fill out the intake form at least 24 hrs prior to your appointment so we can review before meeting. You may submit any relevant blood work that you have for review. Labs greater than 3 months old will need re-testing. Complete lab results will be reviewed with you during your second consultation or once results are received. Follow up visits will be scheduled on an as needed basis depending on treatment. Some patients work with us for a few months while others need much longer to heal.

Important Information for New Patients

LAB TESTS Some labs can be completed locally at Quest or Lab Corp while others like stool, urine, saliva, or blood spot (skin prick) samples can be mailed and completed in the comfort of your home. You will be mailed all needed home test kits at the time of your consult.

SUPPLEMENTS All of the supplements that are recommended by Dr. Macie are available for purchase online through our web store. Supplements purchased online will be mailed directly to you. Dr. Macie will recommend nutritional supplements as part of your treatment program, but you are under no obligation to purchase supplements from our office or website.

CANCELLATION AND RESCHEDULING OF APPOINTMENTS We kindly ask that you cancel within 24 hours of your appointment. You may cancel your appointment by emailing us at naturalfamilymedchiro@gmail or calling the office at (321) 351-0517 or by accessing the appointment confirmation email to change details.

PAYMENTS All credit card payments will be processed the same day of the visit or phone consult.

INSURANCE If requested, we can provide you with an itemized receipt that you can submit to your insurance carrier for potential compensation. We do not accept insurance.

PHONE CALLS AND MESSAGES Voicemail will be responded to within 24 hours (during business hours). To reach the office, call (321) 351-0517. If you call after hours, please leave a message and the office staff will return your call on the next business day. Please include your full name, date of birth, and phone number. If you have a medical emergency, call 911 or go directly to the ER.

EMAIL If you have questions about labs or anything administrative, please email naturalfamilymedchiro@gmail.com. Please note that it can take up to 48 hours to respond to emails particularly if it is the weekend.

PRIMARY CARE Please note that Dr. Macie is not your primary care physician and we recommend that you have a primary care physician for physical exams.
